Carolyn Bornstein graduated from the Faculty of Pharmacy, University of Toronto, and completed a hospital pharmacy residency at Mount Sinai Hospital. She worked for Sunnybrook Health Sciences Centre and is now at Southlake Regional Health Centre. She has practised in many areas of hospital pharmacy, including intensive care, geriatrics, oncology, drug use evaluation, and, most recently, the Arthritis Program, a nationally recognized rheumatology clinic where she works collaboratively to provide patient care and education. She is a member of the award-winning Medication Management Support Service, a home visit program for seniors, including recently discharged patients. In 2011 she qualified as a Certified Geriatric Pharmacist and is a trained facilitator for health care professional workshops aimed at promoting behaviour change among patients with chronic disease.
At Southlake, Carolyn co-led the pilot project to fund a pharmacist on the intensive care unit team. She developed policies and procedures for compounding sterile preparations (including oncology) based on USP Chapter 797. That led to presentations across Canada to share her knowledge and expertise. Carolyn served on the hospital’s Research Ethics Board for 10 years, and she coordinated continuing education programs for her local pharmacist association for 14 years. Carolyn has precepted pharmacy and nursing students. In 2005 she received the E Amy Eck award from the Canadian Society of Hospital Pharmacists (CSHP) Ontario Branch for her annual presentations to grade 3 students on medication safety and the role of the pharmacist. She has been an assessor for the Pharmacy Examining Board of Canada’s objective structured clinical examination (OSCE) since 2004.
Carolyn has been actively involved with CSHP for more than 30 years. She served as Chapter Chair for the Ontario branch and then as Ontario’s National Delegate in 2003. In 2006 she was elected as CSHP National Presidential Officer with the Vision Portfolio. In that role, she oversaw strategic planning for the Society; the creation, launch, and promotion of the CSHP 2015 initiative; and the coordination of the Pharmacy Specialty Networks. In 2007 Carolyn was awarded CSHP Fellow status.

In 2009 Carolyn was appointed Guest Editor for CSHP 2015 on the Editorial Board of the Hospital Pharmacy in Canada Report . As the CSHP 2015 Project Coordinator, she coordinates and facilitates the activities of this pharmacy practice excellence initiative. She represented CSHP at the 2010 Pharmacy Practice Model Summit of the American Society of Health-System Pharmacists. She is currently a trustee for the CSHP Research and Education Foundation.
